Lima, OH: the employer did not ensure established lockout procedures were done in sequence to protect employees tasked with servicing/changing bandsaw blades on the #2 teardown saw. Employees were performing these tasks in an energized state without the benefit of energy isolation and as a result were exposed to caught in/amputation hazards.  b) Lima Pallet Company Inc. on or about March 4, 2024, at the address of 1470 Neubrecht Rd. Lima, OH: the employer did not ensure established lockout procedures were done in sequence to protect employees tasked with servicing/changing bandsaw blades on teardown saw #3. Employees were performing these tasks in an energized state without the benefit of energy isolation and as a result were exposed to caught in/amputation hazards.  c) Lima Pallet Company Inc. on or about March 1, 2024, at the address of 1470 Neubrecht Rd. Lima, OH; the employer did not ensure established lockout procedures were done in sequence to protect employees tasked with duties such as but not limited to belt adjustments and servicing the drive chain on a conveyor. Employees were performing these tasks in an energized state without the benefit of energy isolation and as a result were exposed to caught in/amputation hazards.
