1926.20(b)(2) The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) Ground Level, Parking Lot Center: On or about March 1, 2024, employees were doing concrete forming work around at least 10 pieces of steel rebar that was uncapped and protruding out of the ground, exposing employees to an impalement hazard.
