Location: Job site; On or about 03/01/2024, the employer did not barricade, prohibit or otherwise prevent employees, other sub-contracted employees, or host employer employees from working in an area which had the potential for roofing and construction materials falling from the roof through skylights and skylight openings into the work space of the affected lab and office areas below.
