29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. a) Worksite, 4 Fox Run Ct., Ridge, NY- Employee using a 28' aluminum extension ladder to perform roofing and siding work was not trained to recognize the hazards of working from ladders and the means to minimize the hazards; on or about 3/1/24. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

The employer did not report the in-patient hospitalization to OSHA within the following 24 hours; on or about 3/2/24. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
