Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and amputation hazards while operating metal working press brake:  a) 301 Randolph Ave. Ambler, PA - On or about March 5, 2024, the foot pedal on the Chicago Mechanical Press Brake #7297 was not guarded against accidental activation from fall objects or someone accidentally stepping on it.   Among other methods, one feasible and acceptable method to correct this hazard is to follow the guidelines in the American National Standards Institute (ANSI) B11.3 - 2012, Safety Requirements for Power Press Brakes.  Section 6.3.3.2 states "When provided, a foot pedal of the type that serves as an operator control shall be equipped with a means to prevent unintended press brake actuation."
