29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.  On or about March 5, 2024, at a workplace located at 401 Industrial Park Drive, Reedsville, Wisconsin in Plant 1 and Plant 2; the employer had not developed and implemented energy control procedures to ensure employees performing blade and/or tooling changes, adjustments and/or cleaning  were protected from hazardous energy sources prior to engaging in the servicing and/or maintenance tasks.

29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   On or about March 5, 2024, at a workplace located at 401 Industrial Park Drive, Reedsville, Wisconsin in Plant 1 and Plant 2; employees performing servicing and/or maintenance work, including but not limited to, blade changes, clearing jams and cleaning were not trained in the methods and means necessary for energy isolation and control.

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:   On or about March 5, 2024, at a workplace located at 401 Industrial Park Drive, Reedsville, Wisconsin in Plant 1; the in-running nip point created by the outfeed conveyor belt and the end roller on the resaw was not guarded to prevent a nip point hazard.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about March 4, 2024, at a workplace located 401 Industrial Park Drive, Reedsville, Wisconsin in Plant 1; the multi head drill press was not guarded to prevent employee exposure to the rotating drill heads.

29 CFR  1910.213(h)(1): The upper hood shall completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The upper hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters, broken saw teeth, etc., and will deflect sawdust away from the operator. The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed:   On or about March 5, 2024, at a workplace located at 401 Industrial Park Drive, Reedsville, Wisconsin in Plant 2; the Dewalt radial arm saw utilized in the assembly department was not equipped with an adjustable blade guard to prevent employee exposure to the shear hazard.

29 CFR  1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury:   On or about March 5, 2024, at a workplace located at 401 Industrial Park Drive, Reedsville, Wisconsin next to the Propane Cylinder Storage Rack; the fire extinguisher mounted to the racking used to store LP cylinders created a potential hazard by directing employees to a potential fuel source.

29 CFR  1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected:  On or about March 5, 2024 at a workplace located at 401 Industrial Park Drive, Reedsville, Wisconsin in Plant 1 and Plant 2; the employer did not require employees to perform an examination and/or inspection of forklifts prior to use each work shift.
