29 CFR 1910.28(b)(11)(ii): (ii) Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2) a. On or about March 7, 2024, in the warehouse, employees were exposed to fall hazards greater than 6 feet while accessing the second story of the building from fixed stairway due to a section of missing handrail along an open side.
