29 CFR 1910.146(d)(4)(iii):Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at no cost to the employees communications equipment necessary for compliance with 29 CFR 1910.146(h)(3) and 29 CFR 1910.146(i)(5): At the workplace located at 2000 NW US Highway 24, Topeka, Kansas 66618, employees engaged in performing repair work on the #1 carbon black handling system inside a permit required confined space under tower 1 were exposed to potential oxygen deficient and hazardous atmosphere hazards. The entrants were wearing respirators and were not able to communicate effectively with the attendant above ground.
