29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) On or about March 5, 2024, employees working on and around the Carpenter Mfg. Co. - Tube Cutter (Model No. 42C & Serial No. 4200364) were exposed to an amputation hazard, in-that the employer did not ensure the exit chute was properly guarded to prevent the operator from reaching into the point of operation.
