29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. On or about March 6, 2024 the employer Sam's Welding, Inc., at jobsite located at 3360 Glendale Ave. Toledo OH 43614, did not ensure that employee(s) operating a JLG Lift (Model: 400S S/N: 10827298) articulating mobile scaffold (aerial lift) donned and secured their personal fall arrest systems before operating the aerial lift. Two employees were exposed to falls of approximately 14 feet to the ground below in that a fall protection harness and lanyard was not utilized by the employees when using the mobile scaffold to perform welding activities on a commercial steel erection construction jobsite.
