29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: On or about March 6, 2024, future office area hallway, employees used mixers, hot plate, and lights receiving power from extension cords that were plugged into an outlet. The electrical outlet was not on a circuit protected by a ground fault circuit interrupter.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about March 6, 2024, employees used equipment such as, but not limited to a mixer and hot plate receiving power from flexible cords in the following instances: (a) In the hallway of the future office area, one 100-foot long 12AWG flexible cord was missing the ground pin. (b) In the auto fluids and aerosol storage room, the 10 AWG flexible cord was missing the ground pin.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about March 6, 2024, on the south side of facility from the office area entrance, employees used a Craftsman portable light receiving power from a cord that had exposed inner conductors at the female connection.
