The employees were potentially exposed caught-in hazards as a result.

On or about March 07, 2024, at the grinder location of the facility the employer did not ensure that employees performing the covered activity of cleaning for the Cumberland Grinder, Model 1628X were provided with authorized lockout/tagout training, in order to identify the hazards associated with the cleaning work and the procedures necessary to remove and verify energy isolation measures were effective.
