29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about December 7, 2023, the employer did not ensure that employees were provided with and required the use of appropriate personal protective equipment when they accessed the shut-off valve to the COP tank. Employees were exposed to burn hazards to their head and face from steam being released from the COP tank while turning off the shut-off valve. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
