29 CFR  1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  (a) On or about March 7, 2024,  and dates prior in the service department, the employer did not ensure modifications made by the employer to the mast of the Big Joe (Model # 2018-R6) powered industrial truck which could affect capacity and safe operation of the powered industrial truck had been approved in writing by the manufacture. The Big Joe was modified by welding an attachment hook to the right side vertical mast to hold the golf cart in place when lifted and being serviced by service technicians.
