29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about March 7, 2024, employees conducting roofing work on a 6:12 pitch roof of the residential new construction site were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 12 feet , two (2) inches on the South side of the structure at the eave, and approximately 23 feet at the South side ridge. Hidelia's Development, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number1651076, citation number 1, item number1, and was affirmed as a final order on 08-22-2023 , with respect to a workplace located at 64 York St. Union Grove, WI 53182.

29 CFR 1926.503(b)(1):The employer did not verify the existence of fall protection training by preparing and providing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. (a) On or about March 7, 2024, at a jobsite located at 456 Kiddle Ln, Union Grove, WI 53182 the employer did not verify fall protection training by preparing and providing a written certification record for employees tasked with roof installation on a new residential home where employees were exposed to fall hazards.
