29 CFR 1926.703(a)(1): Formwork was not designed, fabricated, erected, supported, braced and maintained so that it would be capable of supporting without failure all vertical and lateral loads that could reasonably be anticipated to be applied to the form: On or about March 7, 2024, employees engaged in concrete formwork at the Hawkins Construction jobsite at 15705 Harlan Lewis Dr, Bellevue, Nebraska, were exposed to fall and struck by hazards in that the concrete form had not been fastened, braced, or designed in a way that would prevent it from tipping over while employees were on or near it. The result of this failure was two employees riding the formwork to the ground when the formwork failed to remain vertical.
