29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2). Location: 17 Stewart Place, Fairfield, NJ 07004. a: The employer failed to certify in writing that a workplace hazard assessment for equipment that included but were not limited to a Angel Tart Press Machine, Model # E5005M, Serial # 10519E505 was conducted. On October 27, 2023, an employee suffered a right index amputation while operating a tart press machine. Violation occurred on November 2, 2023.
