29 CFR 1910.23(c)(13): The employer did not ensure that ladders are not placed on boxes, barrels, or other unstable bases to obtain additional height: Northside of the building, On or about March 8, 2024 and times prior thereto, employees were exposed to a fall hazard while operating a scissor lift used an A-Frame ladder for additional height to reach their work area.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Northside entrance: On or about March 8, 2024, and at times prior thereto, employees were exposed to electrical hazards associated with unexpected startup or release of stored energy while removing light ballasts that were not locked out.
