29 CFR 1926.703(b)(5): The sills for shoring were not sound, rigid, and capable of carrying the maximum intended load: a) 317 N Market Street Wilmington, DE 19801: On or about March 8, 2024, employees exposed to fall and struck by hazards while conducting concrete formwork activities in that, the sills/footing on which the formwork rested was not sound. Rather it rested on 5.5 inch ×5.5-inch timber that was erected on its end to support 700 series (PS 703) and (PS 706) shoring poles along with Kaufmann HT 20 plus timber. The 5.5 inch ×5.5-inch timber had not been properly braced to meet the design professional's specification. b) 317 N Market Street Wilmington, DE 19801: On or about March 8, 2024, employees exposed to fall and struck by hazards while conducting concrete formwork activity in that, the sills/footing on which the formwork rested was not sound. Rather it rested on 1.5-inch x 5.5-inch lumber that was stacked two high equaling 3 inches x 5.5 inches supporting 700 series (PS 703) shoring poles along with Kaufmann HT 20 plus timber.

Rather it rested on unrated timber pieces of wood stacked above the shore head frame causing the timber to be unstable.
