29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about March 11, 2024, an employee was exposed to machine hazards while performing servicing and maintenance activities and the employer had not ensured the machine specific lockout procedures outlined the specific requirements for testing the machines to determine and verify the effectiveness of the lockout devices. The written procedures did not specify the steps to verify: a. the electrical energy stored in capacitors, hydraulic pressure and pneumatic pressure were de-energized on the CNC Lathe SL25 - Machine 1 and 2. b. the electrical energy stored in capacitors, hydraulic pressure and pneumatic pressure were de-energized on the Mori Seiki CNC CL200 Lathe - Machine 2. c. the hydraulic oil pressure and pneumatic pressure were de-energized on the Fadal CNC Center VMC 6535 - Machine 11. d. the hydraulic oil pressure and pneumatic pressure were de-energized on the Fadal CNC Center VMC 4525 - Machine 12. e. the hydraulic pressure and pneumatic pressure were de-energized on the Alliant Mill - Machine 13. f. the hydraulic pressure and pneumatic pressure were de-energized on the Grob Metal Saw - Machine 14. g. the electrical energy stored in capacitors and hydraulic fluid were de-energized on the Daito Metal Saw - Machine 21. h. the hydraulic oil pressure and pneumatic pressure were de-energized on the Fadal CNC Center VMC 6030 HT - Machine 50. i. the hydraulic oil pressure and pneumatic pressure were de-energized on the Fadal CNC Center VMC 4020 HT - Machine 51. j. the oil pressure to the hydraulic tracer pump was de-energized on the DMG Mori NLX2500/700 - Machine 52. k. the oil pressure to the hydraulic tracer pump was de-energized on the DMG Mori NLX2500/700 - Machine 53. l. the oil pressure to the hydraulic tracer pump was de-energized on the DMG Mori SL-403 - Machine 54. m. the oil pressure to the hydraulic tracer pump was de-energized on the DMG Mori SL-403 - Machine 55. n. the hydraulic oil pressure and pneumatic pressure were de-energized on the Doosan DMN 5700 - Machine 56. o. the hydraulic oil pressure and pneumatic pressure were de-energized on the Doosan DMN 5700 - Machine 57. p. the hydraulic oil pressure and pneumatic pressure were de-energized on the Doosan DMN 5700 - Machine 58.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: On or about March 11, 2024, an employee was exposed to machine hazards while performing servicing and maintenance activities and the employer had not developed a training program in accordance with the standard.
