29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:  (a) On or before 3/11/2024, the employer did not ensure that employees were instructed on the recognition and avoidance of unsafe conditions applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury. Another subcontractor employee was standing on the mid rail of a fully extended scissor lift. The superintended had not walked the site yet and it was past 11 am while subcontractor employees were exposed to life-threatening hazards. These conditions exposed subcontractor employees to trench collapse hazards as well as fall hazards greater than 15 feet onto concrete.
