29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: (a) Department of Veterans Affairs, Lexington, VA Police Area; on or about 03/11/2024, and at times prior, an energized outlet was missing the faceplate exposing employees to an electrical hazard while conducting safety walks checking for jobsite hazards.
