Sugarloaf Mountain Corporation was previously cited for a violation of this occupational safety and health standard 1904.39(a)(2), which was contained in OSHA inspection number 1650576, citation number 1, item number 1 and was affirmed as a final order on 03-03-2023, with respect to a workplace located at 5092 Access Road, Carrabassett Valley, ME.

29 CFR 1904.29(b)(2): The employer did not complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness:  Facility - The employers incident report, which was used in lieu of the OSHA 301 form, did not contain all the information that is required on the 301 form, in the following instances:  a)  The title of the individual who completed the form. b)  Section 4: The date the employee was hired. c)  Section 6: The name of the treating physician or other healthcare professional. d)  Section 8: Location to document if the injured employee was treated in an emergency room. e)  Section 9: Location to document if the employee was hospitalized. f)  Section 10: The case number from the OSHA 300 log. g)  Section 12: The time the injured employee began work on the day of the incident.
