29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  a. On or about March 11, 2024, employees operated foam wire cutting machines in the production area are exposed to thermal hazards at the cutting wires operating at approximately 184-228 degrees Fahrenheit. The employer did not ensure the hazard assessments were conducted for machine operators at the foam wire cutting machines to identify and communicate the hazard and necessary personal protective equipment (PPE) such as, heat-resistant gloves, to protect employees' hands from serious burns.

29 CFR 1910.138(a): General requirements. Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes"  a. On or about March 11, 2024, employees operating the foam wire cutting machines in the production area are exposed to thermal hazards at the cutting wires operating at approximately 184-228 degrees Fahrenheit. The employer did not select and have each affected employee use hand protection to protect from thermal hazards while operating the wire-cutting machines.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  a. On or about March 11, 2024, employees were operating the Walley Contour Cutter machine, which had a 240VAC circuit breaker, that was secured in the closed position with electrical tape wrapped around the enclosure box. The employer did not ensure that the electrical equipment was examined and operating as intended to provide effective overcurrent protection for the equipment.
