29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Location: 1615 51st Street, North Bergen, NJ 07047. a) A NiMCO milk carton making machine, serial number 720GH2117SR789 was not equipped with one or more guarding devices, exposing employees to crushing and entanglement hazards. An employee suffered a laceration on his right hand after attempting to remove a stuck piece of carton from the printer located inside the machine. b) A NiMCO milk carton making machine, serial number 720GH2117SR789 was not equipped with one or more guarding devices, exposing employees to crushing and entanglement hazards. An employee was exposed to entanglement and crushing hazards while standing/working in close proximity to an unguarded chain or belt that ran horizontally along the machine. Violation occurred on or about March 5, 2024.

29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation. Location: 1615 51st Street, North Bergen, NJ 07047. a) The employer failed to implement proper lock out tag out methods prior to performing minor servicing activities on a NiMCO milk carton making machine where alternative measures were not used to protect employees from entanglement and crushing hazards. An employee suffered a laceration on his right hand after attempting to remove a stuck piece of carton from the printer located inside the machine. Violation occurred on or about March 5, 2024.
