29 CFR 1926.20(b)(2): The employer did not initiate and maintain a program that provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer;   a) On March 12, 2024, and times prior the employer did not effectively maintain the site inspection system to identify and correct hazards to which their employees were exposed. Hazards included employees not using fall protection, employees using unserviceable fall protection components and employees using non-compatible fall protection components.   b) On March 12, 2024, and times prior the employer did not effectively maintain the site inspection system to identify and correct hazards to which their employees were exposed.  Employees used pneumatic powered hand tools without proper eye protection.   c) On March 12, 2024, and times prior the employer did not effectively maintain the site inspection system to identify and correct hazards to which their employees were exposed.  An energized flexible cord used for temporary power had damaged insulation and exposed energized conductors.   Note) Certification of corrective action is required for these items.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On March 12, 2024, and times prior employees were exposed to a 12-foot fall hazard to the ground below while performing framing activities on a flat roof deck without their fall protection attached properly to an anchor point or not at all. The shock pack and controlled tear stitching were removed from the lanyard.   Note) Certification of corrective action is required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person qualified in the correct procedures for the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:  a) On March 12, 2024, and times prior an employee used an unserviceable energy absorbing lanyard. The shock pack and controlled tear stitching had been removed.  The employee did not recognize the hazards of using unserviceable personal fall arrest components or the actions needed to correct the hazard.   b) On March 12, 2024, two employees were attached to the same anchor point.  The employees did not recognize the hazards of not properly using personal fall arrest or the actions needed to correct the hazard.   c) On March 12, 2024, and times prior employees used fall protection components that were not compatible, in that rebar hooks were attached to snap-hooks.  The employees did not recognize the hazard of not properly using personal fall arrest or the actions needed to correct the hazard.   d) On March 12, 2024, and times prior an employee used the snap-hook attached back onto the lanyard as part of the anchor point.  The employee did not recognize the hazard of not properly using personal fall arrest or the actions needed to correct the hazard.    Note) Certification of corrective action is required for this item.
