Stairs or a ramp were not provided where the exit route was not substantially level. (a) On or about March 12, 2024, located at 4837 West Woolworth Ave. Milwaukee, WI 53218, the employer did not ensure that an exit door on the east wall near the tear down machinery was unblocked exposing employees to slip, trip, and fall hazards.
