Mobley Rd., Odessa, FL 33556, on or about 03/12/2024, the employer exposed employees to cave-in hazards, in that, employees were working in a trench at depths of over 5 feet from grade with no cave-in protection. Employees had not been instructed/trained on excavation sloping requirements or safety regulations applicable to trenching/excavation work.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) For the trench at/near the corner intersection of Racetrack Rd. and S. Mobley Rd., Odessa, FL 33556, on or about 03/12/2024, the employer exposed employees to cave-in hazards, in that, there was no competent person on site to inspect the trench. Employees were exposed to cave-in hazards while working in the trench at depths of over 5 feet deep with no cave-in protection.

Mobley Rd., Odessa, FL 33556, on or about 03/12/2024, the employer exposed employees to cave-in hazards, in that, employees were working in a trench at depths ranging from 5.25 feet to 5.65 feet with vertical/90 degree side walls (no sloping). No cave-in protection was being used.
