29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about March 12, 2024, employees were exposed to a fall hazard of 30 feet to concrete while replacing the seals around the skylights on the roof of a commercial building located at 2900 Vincent Street, Cuyahoga Falls, OH 44221. The employer did not ensure the employees were protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.

29 CFR 1926.502(h)(1)(v):The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: On or about March 12, 2024, employees were exposed to a fall of 30 feet to concrete while installing new seals around skylights on the roof of a commercial building located at 2900 Vincent Street Cuyahoga Falls, Ohio 44221. The competent person determined by the employer who was acting as the safety monitor was observed and photographed sweeping and had other responsibilities which could take the monitor's attention from the monitoring function.
