29 CFR 1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. On or about March 12, 2024, employees were exposed to a pull boxes, and junction boxes with missing faceplate cover's.

29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses. On or about March 12, 2024, an employee was using a milling machine in the shop area while wearing glasses that did not have side protection.
