Location: Job site;  On or about 03/13/2024, the employer allowed roofing debris, shingles, and a wheelbarrow to be stored within 6-feet of the roof's edge and had not erected a guardrail system to protect employees walking/working in the vicinity of the roof's edge that was approximately 25-feet above the ground below. Furthermore, there was a portable extension ladder erected in the vicinity of the stored equipment and materials which posed an overhead struck-by hazard to employees who ascended and descended the ladder without hard hats.

Luis Caguana DBA LC Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13), which was contained in OSHA inspection number 1559755, citation number 2, item number 1 and was affirmed as a final order on 04/20/2022, with respect to a workplace located at 55 Lodi Rd., Marlborough, MA 01752.
