29 CFR 1910.178(k)(1):The brakes of highway truck(s) were not set and wheel chocks placed under the rear wheels to prevent truck(s) from rolling: Employees are exposed to struck by hazards due to highway trucks are not being chalked prior to the forklifts entering the trailer for load and unload. This could allow the trailers to slide forward when entered, dropping the dock plate and having the forklift fall backwards between the trailer and the dock.

1910.305(g)(2)(ii):Hard-service cord and junior hard-service cord No. 14 and larger were repaired by splicing, which did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced:Note: Hard service flexible cords No. 12 or larger may be spliced if the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. Employees were exposed to shock and fire hazards in that the company allowed had them utilize a Safe Speed Panel Saw system which utilized a Milwaukee rip saw, model 6480-20, while the cord was damaged, and not properly repaired. Protection from the spliced cordage was a wrapping of electrical tape.

1910.305(h)(6):Strain relief was not provided at connections and terminations. Employees were exposed to shock hazards when charging the stand up Hyster forklift, Model N40ZR-16.5 with the Hawker Pro charging station due to the wires being stretched from power cord to the control box allowing the positive and negative wires to be exposed as they enter the metal box.
