29 CFR 1926.1053(b)(1): Because of the portable ladders' length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rails, shall be provided to assist employees in mounting and dismounting the ladder. On or about March 13, 2024, at 2225 N Andrews Ave, Fort Lauderdale FL 33311, the employer did not ensure the 24-foot aluminum extension ladder being used to access the roof from the lower level were secured at the top to a rigid support so that the ladders would not deflect while employees using the ladder that was not secured to access a roof were exposed to a 14-feet fall.
