29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about February 6, 2024, the employer failed to report an inpatient hospitalization as the result of an employee engaged in new construction telecommunications activities by running overhead telecommunications wire in support of commercial construction activities on the job site located at 10730 Pacific Street, Omaha, NE 68114; when on February 5, 2024 at approximately 0915 am the employee fell from a step ladder and was taken and treated at a local hospital. Instances of hospitalization include but are not limited to: i. February 5, 2024, an employee was admitted and treated over a 2-day period. ii. On or about February 14, 2024, an employee was re-admitted and treated over a 4-day period.
