On or about March 14, 2024, the employer did not ensure that employees operating the Hyster R30ES (S/n B174H02447S) and Crown SP3500 (S/n 1A341477) powered industrial trucks were protected from falls greater than 4 feet while performing appliance loading/unloading activities within the distribution warehouse. Employees were exposed to falls of approximately 12 feet which has the potential to cause fractures, contusions and possibly death.

29 CFR  1910.132(a):Protective equipment, including personal protective equipment for the extremities was not provided and used wherever chemical hazards are encountered that are capable of causing injury or impairment in the function of any part of the body through absorption or physical contact:  Farm and Home Hardware located at 140 Center Street in Ashland, Ohio 44805: On or about March 14, 2024, the employer did not provide personal protective equipment for employees tasked with performing service on the Toyota E90D-17 (S/n RWE00390459) powered industrial truck's batteries. Employees added water to the batteries and were exposed to splash hazards from sulfuric acid. Failure to provide personal protective equipment to  employees exposed employees to chemical burns and/or rashes associated with dermatitis.
