29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  Location: 31 Thayer Lane Jobsite back of house on deck  On or about March 14, 2024, employees were exposed to fall hazards while accessing and egressing a ladder jack scaffold without proper means of access.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: Jobsite  On or about March 14, 2024, employees were exposed to fall hazards of approximately 9 feet to the ground below due to PFAS not being properly used.  Instance 1: an employee was engaged in residential roofing operations and was not connected to fall arrest system.  Anchor points for the fall arrest system were not installed per manufacture specifications as shown on the label of the roof anchor.  Instance 2: an employee was engaged in residential roofing operations and was not connected to the fall arrest system.  Anchor points for the fall arrest system were not installed per manufacture specifications as shown on the label of the roof anchor.  Instance 3: an employee was positioned on a ladder jack scaffold and had the rope grab attached to the front waist area of the PFAS Harness.   Instance 4: an employee on a ladder jack scaffold was wearing a PFAS Harness but was not wearing it properly as evidenced by one leg not having webbing around it and not in the harness.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about March 23, 2023, employees were not provided training to recognize or avoid fall hazards. Employees were not wearing a personal fall arrest system and were exposed to fall hazards of approximately 10 feet from a rooftop to the ground below.
