29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items A-D of this section: a) Tool AB06 Area - On or about March 14, 2024, an employee was exposed to burn, electrical shock, and chemical hazards due to the company utilizing the AMAT manual for the energy control procedure. The employee would need to reference different sections in the general procedures in the AMAT manual for a Cryo Pump Replacement for Tool AB06 (AMAT (Varian) Model - Viista 810XPT, SN #: ES138450). The different sections and the the AMAT manual did not allow the employee to go through the energy control procedure in a specific step-by-step process for Tool AB06. The energy sources included 208V pump power, High Voltage Enable: 200KV, N2, and CDA.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Tool AB06 Area - On or about March 14, 2024, an employee was exposed to burn, electrical shock, and chemical hazards due to the company utilizing the AMAT manual for the energy control procedure. The employee performed a Cryo Pump Replacement for Tool AB06 (AMAT (Varian) Model - Viista 810XPT, SN #: ES138450) that had energy sources that included 208V pump power, High Voltage Enable: 200KV, N2, and CDA, along with the Cryo Pump containing chemicals such as Arsine, Helium, Phosphine, Xenon, and Boron Trifluoride.
