29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a) On or about March 11, 2024, employees were exposed to machine hazards associated with moving parts, in-going nip points, while cleaning the upper and lower conveyor belts of the Ultium Cells L&S 8-3 Stacking machine located in the Lamination and Stacking assembly area. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)] were not implemented to protect employees from machine servicing hazards.  shutdown the machine or equipment [(d)(2)];   machine isolation [(d)(3)] - Physically locate and operate all energy isolating devices that are needed to control the energy to the machine or equipment in such a manner as to isolate the machine or equipment from the energy source(s);   LOTO device application [(d)(4)] - Affix a lockout or tagout device to each energy isolating device;   dissipation of residual energy [(d)(5)(i)] - relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy;   verification of isolation [(d)(6)] - Verify that isolation and deenergization of the machine or equipment has been accomplished.

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  a) On or about March 11, 2024, the crew of operators in the lamination and stacking assembly area were tasked with utilizing the safety control system (SCS) to clean the Ultium Cells L&S 8-3 Stacking upper and lower conveyor belts. The employer did not ensure the crew of operators utilized a procedure which afforded the operators a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device. The SCS was not equivalent in that the Ultium Cells L&S 8-3 Stacking upper and lower conveyor belts are able to be moved by an employee (1) at the HMI while the interlock doors are bypassed and an employee (2) cleans the upper and lower conveyor.
