Monmouth Junction NJ 08852 - The employer failed to utilize any traffic control signs or barriers during an evolution where workers were exposed to vehicular traffic struck by hazards. On November 08, 2023 where an employee in an unprotected manhole was struck in the head by a vehicle's tires as they passed over manhole.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, employer failed to report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Esposito Construction, LLC - 211 Henderson Rd. Monmouth Junction NJ 08852 - The employer failed to report the in-patient hospitalization of an employee injured on November 08, 2023, where an employee was in a manhole setting a laser when a civilian vehicle drove over manhole and injured the employee. The employer was aware that the employee was admitted to hospital the day of the injury and did not report to OSHA. Occurred on November 8, 2023.
