On or about: March 13, 2024 Location: 111 Tomkins Ave., Stony Point, NY 10980  a) Two (2) workers accessing pump jack scaffold and back porch roof utilizing extension ladders with lanyard missing. Workers exposed to fall hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about: March 13, 2024 Location: 111 Tomkins Ave., Stony Point, NY 10980  a) Two (2) workers not utilizing fall protection when working at heights greater than 6 feet. One (1) worker on the back porch roof approximately 10-12 feet in height. One (1) worker on a pump jack scaffold approximately 20 feet in height. Workers exposed to fall hazard.  J.G. Home Improvements/Ren Corp. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1705917, citation number 002, item number 001 and was affirmed as a final order on 12/7/2023, with respect to a workplace located at 59 Hudson Terrace, Sleepy Hollow, NY 10591.

On or about: March 13, 2024 Location: 111 Tomkins Ave., Stony Point, NY 10980  a) The extension ladder used to access the pump jack scaffold was extended up to the pump jack scaffold working platform but did not extend at least 3 feet above the upper landing surface. One (1) worker exposed to a fall hazard.  J.G. Home Improvements/Ren Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1705917, citation number 001, item number 003 and was affirmed as a final order on 12/7/2023, with respect to a workplace located at 58 Hudson Terrace, Sleepy Hollow, NY 10591.

Workers were not provided with ladder safety training.  J.G.
