29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about March 13, 2024, at the job site: employees were exposed to an approximate 20 foot fall hazard while gaining access to the 5/12 pitched roof, in that, the extension ladder was only extended approximately 12-18 inches past the roof landing.
