29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): On or about March 11, 2024, at the north side of the building, employees worked from scaffold platforms that were not fully planked when they placed sheet metal on the roof.

29 CFR 1926.451(e)(4)(xiii): Riser height was not uniform, within 1/4 inch, (0.6 cm) for each flight of stairs. Greater variations in riser height are allowed for the top and bottom steps of the entire system, not for each flight of stairs: On or about March 11, 2024, at the north side of the building, a scaffold used by employees as stairs to access the building roof did not provide uniform stair riser height from the scaffold platforms to the building roof.
