29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading. Establishment loading dock: On or about March 15, 2024, the employer did not ensure that a trailer that was being unloaded had its brakes set and wheels chocked. An employee sustained injuries after he and his forklift fell from a loading dock after a trailer truck pulled away from the loading dock, while the forklift operator was still unloading the trailer.
