At a residential new construction worksite located at 3200 S Farm Rd 187, Springfield MO -- employees engaged in roofing duties using pneumatic nail guns and other small hand tools were not wearing eye protection. This exposed the employees to getting debris in their eyes. This was documented on or about 3/18/2024.

The ladder was not extended 3 feet above the landing surface. This exposed the employees to falling from a ladder at height greater than 10 feet to ground level. This was documented on or about 3/18/2024.
