29 CFR  1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a,) On or about March 19, 2024 and within a month prior, employees used a powered industrial truck in the warehouse area without completing training and evaluation.

29 CFR  1910.157(e)(3):The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.  a.) On or about March 19, 2024 Employees in the office area were exposed to fire extinguishers that had no annual maintenance check documented.
