On or about March 14, 2024, at loader rack area, the employer did not ensure safe access between the elevated catwalk and tank trailer(s), as employees open lid to fill tank(s) with fertilizer approximately 9 feet above ground.

29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   On or about March 14, 2024, at loader rack area, the employer did not ensure fall protection was provided when working on top of tank trailer at an approximate height of 9 feet from ground level, as employee(s) are on  tanks to open lid and fill tank(s) with fertilizer.
