(a) TFORCE LOGISTICS EAST, LLC: On or about March 12, 2024, and at times prior, the employer failed to report a fatality of one employee as a result of a work-related incident within eight (8) hours. An employee sustained fatal injuries on March 12, 2024, at approximately 7:30 PM and OSHA was notified by the coroner on March 19, at 10:20 AM.
