29 CFR 1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about March 19, 2024, and times prior to, the employer did not ensure an employee amputation was reported within twenty-four hours. The International Paper was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number1590690, citation number 1, item number 1, and was affirmed as a final order on 9-26-2022, with respect to a workplace located at 1302 W. Carrier Parkway, Grand Prairie, TX 75050.
