29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) On or about March 10, 2024, employees walking up to the motor room of Lift A  were exposed to a fall hazard due to the elevated surface of the catwalk containing an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level.  b) On or about March 10, 2024, employees walking up to the motor room of Lift B were exposed to a fall hazard due to the elevated surface of the catwalk containing an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level.

29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.  a) On or about March 10, 2024, the employer did not ensure that employees utilizing a staircase on Chair Lift A with at least 3 treads and at least 4 risers, was equipped with a stair rail system and handrails as described in 29 CFR 1910.28 Table D-2, in that the staircase had one handrail with two open sides.  b) On or about March 10, 2024, the employer did not ensure that employees utilizing a staircase on Chair Lift B with at least 3 treads and at least 4 risers, was equipped with a stair rail system and handrails as described in 29 CFR 1910.28 Table D-2, in that the staircase had one handrail with two open sides.

29 CFR  1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:  On or about March 19, 2024, in the maintenance shop, oxygen cylinders in storage were not separated from acetylene fuel-gas cylinders by a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour, thereby exposing employees to a fire hazard.
