Graytown OH 43432, did not ensure employees were protected from falls while performing residential roofing activities atop an 8:12 pitch steep sloped residential second level roof deck. Twelve employees were exposed to falls of approximately 16 feet or greater from the peak and falls of 10 feet or greater from the eaves to the ground below.

29 CFR 1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: On or about March 19, 2024 the employer Frontline Roofing, LLC at jobsite located at 565 Graytown Rd. Graytown OH 43432, did not provide a training program for each employee potentially exposed to fall hazards while using a portable multi-function ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Twelve employees were exposed to falls from approximately 10 feet or greater while using an aluminum multi-function ladder to perform residential roofing construction activities.
